BOSTON, MA – July, 2025
Link Health is proud to announce that it has been selected as a recipient of the New Commonwealth Fund Grant, a critical investment in community capacity building and health equity in Boston. This funding supports Link Health’s efforts to collaborate with mission-aligned organizations to drive sustainable, community-led change.
The grant will help Link Health further integrate its model into Boston’s broader collective impact ecosystem, enhancing its ability to deliver public benefits enrollment, health access support, and social care navigation across underserved communities.
“At Link Health, we believe the most lasting change happens when we build power and infrastructure from within communities,” said Ar’Sheill Monsanto, VP of Partnerships at Link Health. “This grant from the New Commonwealth Fund allows us to deepen our collaboration with local partners and help shape a collective agenda that centers equity and sustainability.”
As part of this initiative, Link Health will work closely with other grantees and Boston-based coalitions to align strategies, share resources, and scale solutions that address the root causes of health disparities, such as food insecurity, lack of insurance, and gaps in digital access.
This opportunity also strengthens Link Health’s ongoing commitment to embedding Patient Navigators in healthcare and community settings, ensuring individuals and families are supported in accessing critical services like SNAP, Medicaid, and the Medicare Savings Program.
